Please let us know your child’s name, class and the reason for their absence. Certain illnesses require periods of absence advised by the NHS. This information can be found in the ‘NHS Summary of Recommended Absence for Communicable Diseases’ below:
Your child may require medication on a short or longer term basis. Medication can be administered in school on receipt of the relevant medical form. No medication is allowed to be administered without the correct form. The forms can be collected from the office and are also available below:
